In-network billing will be submitted to your insurer by Advanced Practice Lactation. You are responsible and will be billed for any required deductibles, co-insurance, or co-payments. PLEASE NOTE: Deductibles are applied annually on your plan’s renewal date.

All other medical insurance plans are considered out-of-network. An insurance superbill will be provided that you can submit for reimbursement from your insurance company.

For out-of-network plans, payment in full is expected in cash, check, or credit card at the time of service.
